You will need to print and mail or fax the form to the appropriate County office. If faxed in, there must be accompanying credit card information for payment. Those forms which have “pdf” beside them are in Adobe pdf format and must be downloaded,printed, filled in and mailed. The address for the specific department will be located at the bottom of each form.
